Before You Register for School Athletics

STEP 1: PHYSICAL EXAMS
Student athletes must have a current physical (within 365 days), on file in the nurse's offices in order to participate in school athletics. If the student's physical expires during the season, they will not be able to play until a new physical exam is received. The Preparticipation Physical Evaluation form must be submitted via hard copy only.

As required by the Scholastic-Athlete Safety Act, effective with the start of the 2015-2016 school year, the physical evaluation may ONLY be completed by a licensed physician, advanced practice nurse (APN) or physician assistant (PA) that has completed the Student-Athlete Cardiac Assessment professional development module. Parents MUST fill out the first page, the Health History of the Preparticipation Physical Evaluation Form. The clearance and physical form must be filled out by your student's medical home. Should your child have special needs, please fill out the special needs form and include it with the physical. The link for the form can be found below.

STEP 2: ATHLETIC REGISTRATION

Please complete registration on Family ID. Athletic registration must be completed by a parent or guardian of the student athlete.
